I．はじめに

　近年，頭部外傷は増加の一途をたどり，社会問題にさえなり，ことに頭蓋内出血の救命手術が諸家の関心を集めている。しかし，その主力はもつばら硬膜上下の血腫にむけられ，脳内血腫が見すごされることも少なくないと考えられる。脳内出血に関する文献ならびに報告は確かに少ないが，けつしてまれではなく閉鎖性頭部外傷の9.5％に達するとの報告^35）もある。近年，本邦においては特発性脳内血腫の治療対策のみに目がむけられて，外傷性脳内血腫に対する関心が少ないように思われる。これらは開頭手術により救命し得るものであることと，われわれの教室でも遭遇する機会が多くなつてきたので，われわれの手術した12症例を中心に外傷性脳内血腫の臨床につき若干の考察を加えて見た。

Twelve cases (2.2%) of traumatic intracerebral hematoma among 540 in-patients of head injuries were operated in the past 7 years.

In this report we presented and discussed about clinical manifestations, neurological signs, examinat-ions, treatments and prognoses of these examples in detail. For diagnosing and treating the intracerebral hematoma accurately, it is important to differentiate it from a diffuse contusional lesions and brain edema. The carotid angiography is the most valuable diag-nostic procedure and evacuation of the hematoma is the most effective treatment.
