視神経脊髄炎の1剖検例

An Autoptic Case of Optic Neuro-Myelitis

　急性脊髄炎を伴つた視神経炎はAllbutt^1）により初めて記載され，ついでErb^5），Segium^15）等の報告もあつたが，脊髄症状と眼症状が偶発的に，同時に発生したものと解釈されていた。その後Devic^4），及びGould^8），Beck^2）等の詳細な研究により，視神経脊髄炎なる概念が確立され，特殊な1疾患として独立した。然しその本態とか原因については現在尚論議されている所で，一定の結論に達してはいないが，脱髄性脊髄炎の1つで，多発性硬化症の一亜型，或は不全型であるという説が有力である^{3）6）7）11）13）14）20）}，最近吾々は結核性脳膜炎と考えたが，10数日の経過で死亡した患者を剖検し，視神経脊髄炎であつた事を確めた1例を経験したので報告する。

Here is a case in which a patient died in an acute process, making the main com-plaints of high fever, headache, nausea, vomiting and disturbances of the sensibility of the both lower extremity.

Holding an autopsy in this case, we diag-nosed it as an optic neuromyelitis, from our pathoanatomical findings as hereafter provided for.

1) Serious inflammation was seen in the cerebrospinal meninges.

2) Hyperemia, attended with an evident demyelinisation, and perivascular cell infil-tration were seen in the spinal cord and the optic nerve.

3) The same findings as mentioned above (2) were seen in medulla oblongata, pons and cerebrum.
