精神運動障害によるメランコリア(内因性うつ病)の鑑別—日本語版CORE尺度の紹介

Distinguishing Melancholia by Evaluating Psychomotor Disturbance:Japanese version of the CORE measure

抄録　CORE尺度とは，抑うつ病態のなかからメランコリア(内因性うつ病)を鑑別するために，制止・焦燥といった精神運動障害を評価する尺度である。「メランコリアの中核的特徴は精神運動障害であり，それを患者の主観的な訴えではなく，観察可能な行動学的特徴として捉えることでメランコリアの識別が可能になる」というParker Gの考えに基づいて開発された。CORE尺度は，メランコリア診断のための補助的情報，あるいは電気けいれん療法の効果を予測する指標などとして有用性が期待される。本稿では，原著者の許諾を得て作成したCORE尺度の日本語版を，使用方法とともに紹介した。

　The CORE measure was designed to distinguish melancholia from other depressive syndromes by evaluating psychomotor disturbance(PMD)such as retardation and agitation. This measure was developed by Gordon Parker. He considered that the core feature of melancholia was PMD, and that melancholia could be identified if PMD was regarded as observable behavioral signs rather than subjective symptoms. The CORE measure provides supplementary information on the diagnosis of melancholia and can be an index to predict the reactivity of electroconvulsive therapy. We prepared a Japanese version of the CORE measure with the permission of the original author.
