アルコール酩酊下における自殺—抑うつ型と攻撃性反転型

Suicides in Alcohol Intoxication: Type of the depression and that of the aggression turned back against oneself

　抄録　自殺とアルコールとの関係については，従来慢性中毒やアルコール症との関係が研究上重視されてきた。急性中毒（酩酊）は専ら暴力犯罪との関係から論じられ，これと自殺の問題についての研究は未だ未解明の諸問題が多く，研究も十分ではない。本論において我々は刑事事件の精神鑑定例からアルコール酩酊下で自殺を計った6症例，7事例について酩酊下で放火した他の2症例と比較しながら主に検討した。これを次のような2群に分類した：A賦活群，B突発群。後者の突発群の特徴である，酩酊時に突然生じる自殺の解明を試み，後者の群に従来指摘されてきた「抑うつ型」の亜型以外に「攻撃性反転型」の亜型があることを明らかにした。この特徴は，（1）複雑酩酊状態で激しい興奮と他者への攻撃性が出現する。（2）攻撃対象人物が本人以外突然全員消失する。（3）この状態と状況で攻撃性が自己に向かい，自殺に至る。また攻撃性の転換に他者（家族）→家屋→自己へ向かう方向性の順序があることを指摘した。（4）この「攻撃性反転型」自殺における自宅放火の意味と役割の重要性と親和性について触れた。

　Alcohol intoxication has been studied especially from the view point of crimes of violence. However sometimes alcohol intoxication has been regarded as one of the main causes of sudden bursts of suicidal impulse, which the subject would never have experienced while sober. The depressive mood in pathological intoxication (Heilbronner), and intoxication complicated by depressiveirritability (Binder) are examples of such causes leading to suicide. But sudden burst of suicidal impulse is observed not only in such depressve states, but also in vital excitement and agression accompanying abnormal intoxication.
