　術前に心エコー図で腫瘍内に嚢腫様透亮像を認めた77歳の女性の左房粘液腫の1手術治験例を経験したので報告する．心エコー図で内部に嚢腫様透亮像を認めた粘液腫はまれであり，現在までに3報告例を認めるのみである．本例は心房細動発作をきっかけに心エコー検査にて診断された．手術は左房，右房縦切開法にて行い，心房中隔に茎を有する30×27×60mm，重さ40gの粘液腫を摘出した．術後は重大な合併症もなく経過順調であった．本邦での70歳以上の高齢者の本手術報告例は20例に満たないが，手術成績，長期予後ともに良好である．

We reported a case of a 77-year-old female, whose left atrial myxoma with a cyst, which had been identified by 2D echocardiography, was successfully resected. Apart from echocardiogram-revealed tumor hemorrhages in atrial myxomas, reports of echolucency within cardiac tumors are considerably rare. She was successfully operated on, and the section through the tumor speci-men showed a cyst with a diameter of 10 mm, that corresponded to the echolucencies noted on the 2D echocardiogram. Even in an elderly patient, surgical treatment of left atrial myxoma offers exellent results with low morbidity and low mortality.
