Abstract 文献概要
1ページ目 Look Inside

　患者は73歳，男性．息切れ，腹痛を主訴に来院．感冒様症状5日目に心不全出現し，当科入院となった．入院時，胸部X線写真で両肺野にうっ血を認め，心電図は左軸偏位，心房細動，不完全右脚ブロックであった．入院5日目，心電図は洞調律，完全右脚ブロックに変化した．入院8日目，胸痛発作あり，心電図で完全房室ブロック，接合部調律，Ⅱ，Ⅲ，aV_FでST上昇，V_4-6でST低下を認めたため，緊急冠動脈造影を行ったが，冠動脈には有意狭窄は認めなかった．

　心エコーで顆粒状エコーを認め，心筋疾患が疑われたため，死後，経皮的に心筋組織を採取した．組織学的に，心筋の広汎な壊死，好酸球，組織球の浸潤，多核巨細胞を認めたため，巨細胞性心筋炎と診断した．巨細胞には形態的に心筋細胞と似ているものもあり，心筋細胞が免疫反応の場となっていることが示唆された．

A 73 year old male patient with a history of pulmo-nary tuberculosis was admitted to our department because of dyspnea and abdominal pain. The chest X-ray film on admission showed bilateral lung congestion. The ECG showed atrial fibrillation, left axis deviation and incomplete right bundle branch block. Five days after admission, the ECG changed into sinus rhythm and complete right bundle branch block. Eight days after admission, the patient complained of chest pain and the ECG showed ST elevation in Ⅱ, Ⅲ, aV_F, reciprocal ST depression in V_4-6, and complete A-V block with jun-ctional rhythm.
