ブレオマイシン実験肺線維症に対する喫煙の影響

The effect of cigarette smoke exposure on the development of bleomycin-induced pulmonary fibrosis in hamsters

　われわれは先に，特発性間質性肺炎において，喫煙者では非喫煙者に比して肺野の縮小が軽度で，全肺気量，静肺コンプライアンスも有意に高値を示すことを報告し^1），喫煙は線維症肺の伸展性を増加させる方向に作用することを示唆した。喫煙は，ヒトの慢性肺気腫の原因として最重要視されているものであるが，喫煙のみで肺気腫を作製することは困難である^2〜4）。しかし，喫煙がエラスターゼ気管内投与による肺気腫の共同因子として作用する可能性が示唆されており^3），喫煙者の線維症肺では線維化のみならず気腫化も同時進行している可能性がある。

　われわれは，ブレオマイシン実験肺線維症モデルを用いて，喫煙が肺線維症発生にどのような影響を及ぼすかを検討し，ブレオマイシンが，タバコ煙曝露ハムスターに線維化のみならず気腫化をも惹起することを，その発生の機序についての文献的考察を加えて報告する。

In this study, we evaluated the effect of cigarette smoke exposure on the development of pulmonary fibrosis induced by bleomycin. Intratracheal bleomy-cin produced pulmonary fibrosis in hamsters, but the exposure of bleomycin-treated animals to cigarette smoke also produced emphysema. Cigarette smoke alone developed no detectable histopathologic evid-ence of emphysema. In hamsters received cigarette smoke and bleomycin, the pressure-volume curve was shifted significantly upward and to the left of that of the bleomycin-treated group, and almost returned to that of the control group. These results suggest that intratracheal administration of bleomycin induced not only fibrotic but also emphysematous changes in the lungs of smoking hamsters.
