　急性心筋梗塞症は年々増加の傾向にあり，50〜60歳ではその病因として冠動脈硬化症が，20〜30歳代の若年者では冠動脈攣縮の関与が示唆されている。我々は34歳で発症した急性心筋梗塞症において，左房粘液腫による冠動脈塞栓が主病変となったと思われる一症例を経験したので，心臓超音波法の診断上の有用性を含め，若干の文献的考察を加え報告する。

A case of a 35-year-old male with left atrial myxo-ma complicated with acute myocardial infarction was presented. Although peripheral arterial em-bolism is frequently encountered in patients with left atrial myxoma, coronary artery embolism has rarely been reported. Coronary angiography in this case did not reveal any degree of narrowing, obstruc-tion nor aneurysmal dilatation of coronary arteries which accounted for characteristic angiographic findings in left atrial myxoma. Recanalization or resolution of embolus derived from LA myxoma into the coronary artery must have occurred before the angiography.

Echocardiography did play a great role in the diagnosis, and its clinical importance was emphasized not only for evaluation of cardiac function but also for determination of the causative abnormalities in acute myocardial infarction.
