Fast Fourier Transform（FFT）アナライザーを用いた人工弁閉鎖音周波数スペクトル分析法

Use of fast Fourier transform (FFT) in the frequency spectroanalysis of the closing sound of the prosthetic valve in the patient

　人工弁置換患者の長期管理における血栓弁，pannusの形成，石灰化などによる弁機能異常では早期発見が診断上重要であるが，機能異常の早期診断法は確立されていない。早期診断からみた心音の変化は機能異常の情報を十分に含んでいると考えられる。これまでにも機械弁置換患者の心音には弁の開閉による高周波成分が含まれているものの，血栓の付着により1KHz周辺の周波数成分が欠落すると報告されている^1）。著者らは人工弁の開閉音周波数スペクトルの変化に基づく異常検出装置を試作し，若干の臨床例で基礎的検討を行い，人工弁閉鎖音に自然心音にはみられない2〜5KHzの周波数成分が含まれ，その周波数スペクトルが特徴的形状を示すことを報告した^2）。また，同時に5KHz以上の成分をも含む可能性も示されたので人工弁音の分析周波数帯域を10KHzまで広げ装置の有用性について臨床的検討を重ね，閉鎖音について聴取部位，置換弁の種類，置換部位による弁音スペクトルの特徴を調べた。本装置を用いて同一患者の同一部位における弁音スペクトルの経時的変化の追跡も加え，本分析装置の有用性を認めたので，本論文ではとくに正常機能弁音のスペクトルパターンについて報告する。

Spectral phonocardiogram of the prosthetic heart valve is considered to give informations of the first signs of prosthetic malfunction such as the thrombosed valve and pannus formation. We made a computerized FFT spectrum analayzing system for the valve closing sound and put it into clinical application to determine featured spectra. Using this system, we obtained normalspectral patterns of the closing sounds which were peculiar to Starr-Edwards ball valve, Starr-Edwards disk valve, Bjork-Shiley valve, Hall-Kaster valve, St. Jude Medical valve, Ionecu-Shiley bioprosthesis and Carpentier-Edwards bioprosthesis.
