ニトログリセリンの左心機能におよぼす影響について

Effect of nitroglycerin on left ventricular performance studied by echocardiography

　狭心症に対し，Nitroglycerin　（NTG），Amylnitrite等の亜硝酸剤が有効であることは古くより知られている^1,2）。一方近年，強心配糖体や利尿剤のみでは治療しきれない難治性心不全に対し，NTG，Isosorbide di—nitrate^3〜15），α—blocker^{12,13,16〜19）}，Hydralazine^{13〜15,20）}，Sodium nitroprusside^{11〜13,19,21〜24）}，Prazosin^24,25）等の諸種血管拡張剤が使用されるようになり，その効果が注目されている。その意味でこれら薬剤の左心機能に及ぼす影響を知る事は重要な事であるが著者らは本態性高血圧症および虚血性心疾患に起因する心機能障害例に対し，血管拡張剤の一つであるNTGを投与し，その収縮期および拡張期動態におよぼす影響を心エコー図を用い検討したので報告する。

The homodynamic response to sublingual administration of nitroglycerin (NTG) were estimated by echocardiography in 13 normal subje-cts (Group-A) and 24 patients with hypertension and/or ischemic heart desease. The patients were divided into two groups to physical capacity of NYHA; 18 patients with NYHA functional class I or II (Group-B) and 6 patients with III or IV (Group-C).

Left ventricular volume were calculatde by cubic method and left ventricular compliance were measured in 10 cases by Smith's method.
