Clinical usefulness of a new patch form of nitrogly-cerin (NT-1), containing 5 mg of nitroglycerin in a patch of 5 cm×10 cm in size was evaluated in 82 patie-nts with heart failure at 38 hospitals in Japan. The breakdown of the cases was ischemic heart disease (52 patients), systemic hypertension (3), mitral or aortic regurgitation (8), myocardial disease (15), and others (4). One or two patches were applied to the chest wall. Symptoms such as dyspnea, coldness of limbs improved. Pulmonary capillary wedge pressure (PCWP), pulmo-nary arterial pressure, pulmonary vascular resistance, blood pressure, systemic vascular resistance and double product decreased significantly (p<0.05~4.01), but heart rate and cardiac index were not changed. A significant reduction in PCWP was observed at 0.5 hours (hrs) after application, and the reduction reachedmaximum at 4 hrs and remained significant through 12 hrs. Clinical effectiveness was confirmed in 41% of patients. Side effects such as headache, skin irritation, decrease in blood pressure were noticed in 15 out of 104 cases studied. Use of the NT-1 had to be discontinu-ed in 4 patients due to decrease in cardiac output or blood pressure.
