　右心房内における選択的結節間伝導路およびBachmann束は解剖学的^1）にも電気生理学的^2）〜4）にもその存在は証明され，心房における興奮伝播過程に重要な役割を果している^5）。

　これら伝導路の障害による興奮伝播過程や心電図の変化は実験的には詳細に検討されているが，臨床的には報告例がきわめて少なく^6）〜9），そのうえ実証的根拠がないため十分解明されたとはいい難い。また報告例はいずれも壮老年者であり，乳幼児例についての報告は著者らの知る限りではこれまでに1例もない。

A rare case of a 4-year-and-9-month-old boy with inter-atrial and intra-atrial block is presented. He was referred for evaluation of an arrhythmia first detected at four months of age. Since then he was periodically followed up and was essentially asymptomatic for about 4 years. Electrocar-diograms showed broad and biphasic P waves with positive and negative deflections in leads II, III, _aV_F and V₁ to V₆. The interval between these deflections, the duration of the P wave and the PQ interval were markedly prolonged.
