Coronary Angiography and Left Ventriculography of Complete Left Bundle Branch Block

　心電図上完全左脚ブロック（CLBBB）を示すのは虚血性心疾患が最も多く，大動脈弁疾患，原発性あるいは続発性心筋症，ある種の先天性心疾患があげられている。また稀ながら有為の心症状あるいは既往歴を持たずCLBBBのみを示すものもあり，その中には家族例の報告もある^1）。

　CLBBBは血行動態への影響はそれ自体で少なく^2）基礎疾患の鑑別が重要である。CLBBBが生ずると心電図上2〜3の特徴があげられてはいるが，心筋硬塞によるQ波との鑑別は難かしく部位診断はもちろん硬塞の診断そのものが非常に困難である^3）4）。心筋症の中にもいわゆるChest pain syndromeを呈するものは比較的多く，虚血性心疾患と心筋症の臨床的鑑別は困難となっている。

Among 650 patients who underwent coronary angiography and left ventriculography at our institute, 6 patients showed complete left bundle branch block (CLBBB). According to their characteristics, 6 cases classified into 3 groups.

Group A. Patients with both normal coronary angiogram and left ventriculogram.

Group B. Patients with normal coronary angiogram and abnormal left ventriculogram.

Group C. Patients with stenotic coronary angiogram and abnormal left ventriculogram.
