高血圧老人の動脈弾性

A study on the elasticity of artery in the hypertensive aged.

I．緒言

　著者は先に大動脈及びその主要分枝動脈の弾性及び抵抗を推測するConwayの簡易法について報告した^1）。本法の原理は，亜硝酸塩（Nitrites）投与に際し，心拍出量が一定不変の場合に，Ni—trites投与前後の脈圧推移から動脈弾性の状態を推測しようとするものである。この際Nitrites投与前後の脈圧推移と血圧因子（Wezler-Boger）との関係をWindkesseltheorieの立場から理論的に考察すると（第1表）に示す如くとなる^1）即ち大動脈及びその主要分枝動脈の弾性低下と共に，投与後の脈圧増加度はより顕著になるといえよう。

　ところで（第1表）において問題になることは，可逆性W型，E'型，（不可逆W＋E'）型の3型の脈圧推移に著しい兼異が認められないという事実である。尤も之には（第1表）の基礎となる理論的考察が極めて概念的であるということも関与するであろうが，併し乍ら臨床上之等3型の分離は，高血圧ないし動脈硬化の検索上，看過し得ない極めて重要な問題を示唆するものと思われる。というのは，之等3型の分離は，高血圧因子の質的，量的判別とともに，動脈硬化の初発部位決定，換言すれば，動脈硬化が末梢性に進展したものか，あるいは反対に中枢性に進展したものかという極めて重要な問題に当然関連してくるからである。

The change of pulse pressuse after the administration of nitrites was examined in the healthily-looking hypertensive aged by Conway's method.

The experiment were perfomed on the intensity of atherosclerosis of thoracic aorta by chestroent-genogram and also made on the intensity of systemic arteriolosclerosis refering on retinal findings, renal function and petechial findings.

It is supposed that the analysis of the hypertension and the detection of the primary focus of arteriosclerosis were recognized more accurately due to the Conway's method, chestroentgenogram, retinal findings, renal function and petechial findings.
