要旨　患者はてんかんによる意識障害で救急搬入された80歳代の女性で，意識が回復すると胸痛を訴えた．心電図ではⅠ，aV_LおよびV_1～3誘導でST部分の上昇が認められ，心臓超音波検査では高度な壁運動異常が認められた．冠動脈造影では有意な狭窄病変は認められなかった．左室造影では心室中部，右室造影では心尖部が無収縮であった．2週後の左室および右室造影では両心室の壁運動異常の改善が認められ，4週後には両心室の壁運動異常は正常化した．本症例では一過性の左室の壁運動異常は中部で認められたため，たこつぼ心筋症(中央部型)と考えられた．さらに，右室にも心尖部で一過性の心筋障害が認められ，このような病態は非常に稀と考えられたため報告する．

　An 83-year-old woman had an epileptic seizure and was admitted to our hospital. After a few minutes she regained consciousness and complained of chest pain. An electrocardiogram showed ST elevation in Ⅰ, aV_L and V_1-3 leads. Though emergent coronary angiography showed no arterial stenosis or spasm, ventriculography confirmed the presence of mid left ventricular and apical right ventricular ballooning. We diagnosed the patient as biventricular Takotsubo cardiomyopathy triggered by epilepsy. Two weeks later, the right wall motion returned to almost normal as shown by ventriculography. After 4 weeks, the left wall motion returned to normal as shown by ventriculography.
