要旨●H. pylori由来の慢性萎縮性胃炎の拡大内視鏡像は胃底腺粘膜では円形〜楕円形の開口部を形成するが，胃底腺の消失した萎縮粘膜では腺窩は溝を形成し，その間の窩間部が管状模様を呈する．しかし，A型胃炎では萎縮粘膜においてもやや大型の円形や楕円形の開口部が密に配列する像を呈する傾向を認めた．近位側で萎縮が著明な胃は通常観察でもA型胃炎と容易に診断できるが，そのような所見を伴わない胃では通常内視鏡像からの診断は困難である．しかし，前述の拡大内視鏡像からA型胃炎と診断できた症例を経験した．それは胃炎の発症機序が異なるため腺窩上皮の改築に違いが生ずるためと考えられる．

　Magnifying endoscopic findings of Helicobacter pylori-induced chronic gastritis show a ridged surface structure ; however, magnifying endoscopic findings of type A gastritis show closely arranged, small, round, oval pits. Typical conventional endoscopic findings of type A gastritis show prominent atrophy on the proximal site, but not on the distal site. However, type A gastritis without this typical finding is difficult to be diagnosed using conventional endoscopy. In these difficult cases, the abovementioned magnifying endoscopic feature is practical. In this study, these characteristic findings are discussed.
