要旨　A型胃炎を伴わない多発性胃カルチノイドの1例を報告した．患者は42歳，男性．胃集団検診で発見され，手術目的で当科に紹介された．胃X線検査，胃内視鏡検査では胃角部小彎に線状潰瘍瘢痕を認めるが，カルチノイドとしての描出は困難であった．摘出標本の組織学的検索では胃中部の線状潰瘍瘢痕の口側に，腺境界に沿って最大2mm径のカルチノイドが24個多発していた．胃底腺の萎縮は軽度で，いわゆるA型胃炎ではなかった．従来報告されているA型胃炎合併多発性胃カルチノイドでは高ガストリン血症のtrophic actionがカルチノイド発生に関与していることが示唆されている．しかし本例は多発性胃カルチノイドでありながらA型胃炎を伴わず，カルチノイドの配列にも特徴が認められ，貴重な症例と考えられた．

　A case of multifocal gastric carcinoid without type A gastritis is reported. A 42-year-old man without symptoms underwent upper gastrointestinal x-ray examination for an annual health check-up. Upper gastrointestinal x-ray and endoscopic examinations revealed a linear ulcer scar at the angle. There was no characteristic feature of the gastric carcinoid. Pathological examination of the biopsy specimen from linear ulcer scar showed a carcinoid. A partial gastrectomy was performed. Microscopic examination of the resected stomach disclosed 24 small carcinoid lesions, two of which invaded the submucosal layer along the transitional zone. Although multifocal gastric carcinoids are usually associated with type A gastritis, we could not detect any evidence of type A gastritis in this case.
