Screening Endoscopy for the Duodenum

要旨●近年になり内視鏡技術の向上，機器の進歩および内視鏡医の認知度の上昇により十二指腸病変の報告が増えてきている．病変の拾い上げにはスクリーニング検査が重要であり，検査技術の向上も不可欠である．検査の際にまずは十二指腸の解剖学的特性を理解し，適切な前処置，スコープ選択をする必要がある．そのうえで鎮静剤の使用を考慮する．進行した十二指腸病変に対する治療は部位によって治療難易度や侵襲が大きくなることがある．想定される十二指腸病変の特徴をよく理解したうえで，適切な手順で見落としのない検査を行うことが重要である．部位的に頻度の高い疾患ではないため，今後は症例を集約することで診断学・治療方針の確立などを行っていくことが急務であると考える．

　A duodenal lesion is a rare disease, and screening endoscopy is important for its detection. Recent improvement in endoscopic technologies may lead to better recognition of duodenal lesions, i.e., treatments can be provided earlier. Treating advanced lesions presents a greater challenge, and thus timely lesion identification is important for providing effective therapy. Upon examination, it is important to understand the characteristics of duodenal lesions in addition to deciding the premedication, choice of scope, and considering the use of sedation. There are currently no established criteria for diagnosis and standard treatment. Standardization is considered to be important for accurate diagnosis and the application and further development of treatments.
