胃微細レリーフ像について

Ⅰ．まえがき

　胃X線撮影において胃病変を正しく診断するためには是非とも胃微細レリーフ（胃小区）像の撮影が必要である．

　しかしながら，単に胃レリーフ撮影を行なうだけでは良い微細レリーフは描出されない．特に表層性胃炎，肥厚性胃炎，消化性潰瘍等の場合には不良である．これらの場合には胃内貯留液過多，過酸，粘液，壊死物質，胃壁過緊張等の諸因子が悪影響を及ぼしているものと考えられる．

　To make exact diagnosis of the stomach diseases, it is necessary to take the X-ray photograph of fine gastric relief (small gastric area). To eliminate bad conditions; for instance, too much retention of gastric juice, hyperacidity, mucous, necrotic substances and hypertonicity of stomach wall; preparations before the examination were done. The drugs to intercept the parasympathic nerve, the digestive enzymes and the neutralisation drugs of acid were given and the good image was obtained. In 114 cases these examinations were applied during past seven years in our clinic. The summary of the examination is as follows:

　1) In case of superficial gastritis, hypertrophic gastritis, gastric ulcer and gastric cancer, it is necessary to do the preparation before the examination to get good relief image.

　2) Without the preparation, the fine relief image could be obtained in 20～40% cases in case of normal stomach and atrophic gastritis but in only 10% cases, good image was obtained in case of superficial ＆ hypertrophic gastritis, gastric ulcer and gastric cancer. The part where the fine relief appeared, was only the gastric antrum. But by the preparation before the examination, the minute relief appeared in 40～70% cases and the part of appearance were not only the antrum but the gastric corpus.

　3) It is valuable to diagnose early gastric cancer and small gastric ulcer etc. to take this image with the perparation.

　4) This preparation is also useful for the endoscopical examination of stomach.
