仮称“非特異性多発性小腸潰瘍症”

“NON SPECIFIC MULTIPLE ULCERS OF THE SMALL INTESTINE”

Ⅰ．はじめに

　著者等は昭和38年1月から42年12月までの5年間に，きわめて類似した臨床的病理学的特徴を有する6例の小腸病変を経験した．この一群の疾患は主として若年者を侵し，臨床的には長年月に亙る潜出血と，それによる二次性貧血を主症状とし，低蛋白血症，発育障害，種々の程度の腹痛を伴っている．病理学的には主として回腸中～下部に形成された浅い潰瘍と潰瘍中心性にみられる非特異性炎症像が基本的な病像であった．

　1932年，Crohn¹⁾等によって提唱された新らしい疾患概念が，その後いくつかの曲折を経つつも，現在限局性腸炎として病理学的にも一疾患単位として認められている所以は単に腸管を局所性に侵す非特異性炎症群という雑多なものの総合としてではなく，そこに出現した所見の組合せや，所見の強弱の間に，本症を貫ぬく一連の特徴像があるからにほかならない．

　かかる基本的観点から，著者等は経験6例の小腸潰瘍性病変につき，その臨床像，病理像を記載するとともに，限局性腸炎との差異を比較し，本疾患の位置づけを試みるのを本稿の目的とした．

　The present study is designed to describe clinical and pathological details of six cases characterized by non-specific multiple ulcerations of the upper, and a part of the lower, ileum.

　The most outstanding clinical manifestation common to all these cases is long history of persistent intestinal bleeding with secondary anemia, accompanied by hypoproteinemia, retardation of growth, and varying degrees of abdominal pain. Although some of these signs and symptoms may resemble those of ileojejunitis, there is some disparity between severe intestinal bleeding in these cases and the clinical pictures of regional enteritis. Discrepancy is also seen in the pathological findings, since the latter is characterized by marked proliferative reactions, as manifested by the predominance of tubercle-like granuloma and pronounced fibrous changes, while in all of six cases here presented fibrosis of the submucosa is not prominent and tubercle-like granuloma is nowhere to be seen.

　On the basis of these clinical and pathologiacl observations, it is suggested that these cases should be called as “non-specific multiple ulcers of the small intestine” as distinguished from regional enteritis until proved to the contrary.
