要旨 今回われわれは,胃癌の進展と癌細胞が発現する糖鎖の関連を,約400症例の胃癌について17種類のレクチンと8種類の単クーロン抗体を用い組織化学的に検討した.また2種類の抗p53蛋白単クローン抗体を用いた検索も同時に行った.その結果,胃癌の進展と糖鎖発現との間には,未熟糖鎖と関連する深達度→リンパ節転移のルートと,既存の糖鎖や新生糖鎖などの成熟型糖鎖が関連する肝転移,腹膜転移のルートがあると考えられた.前者に関わる代表的糖鎖はTn抗原で,ABH血液型糖鎖やLewis型糖鎖は後者に属した.Tn抗原発現とp53蛋白発現には統計学的に有意な関連がみられた.
We investigated the relationship between the progression of gastric cancer from early to advanced stage and the expression of carbohydrate antigens of cancer cells. Formalin-fixed and paraffin embedded sections from approximately 400 cases of primary gastric cancer were used. The detection of carbohydrate antigens was performed by avidin-biotin-peroxidase complex (ABC) methods using 17 kinds of biotin-labeled lectins (GS-I-B4, Jacalin, MPA, HPA, VVA, SBA, DBA, RCA-1, RCA-2, ECA, Lotus, AAA, UEA-1, PHA-L, WGA, ConA), anti-T (Thomsen-Friedenreich antigen), Tn, sialyl Tn antibodies, anti-blood group type A, B, and H (2) antibodies, and anti-Lewisx and anti-sialyl Lewisx antibodies. The expression of p53 protein was also investigated by immunohistochemistry using BP53-12 and PAb240 monoclonal antibodies. The results indicated that the carbohydrate antigen expression is profoundly associated with the progression of gastric cancer, based on either immature type of carbohydrates such as T and Tn antigens or mature type of carbohydrates such as ABH blood group antigens and Lewisx antigens. The former was related to the depth of invasion and lymph node metastasis, whereas the latter was related to hepatic and peritoneal metastases. The p53 protein expression was correlated positively with Tn antigen expression, especially in poorly differentiated type of gastric cancer. These results suggest that the carbohydrate deviations of cancer cells are one of the major causes of the progression of gastric cancer from early to advanced stage. The carbohydrate deviations may be due to p53 mutation in gastric cancer cells in part.
