要旨　1967年1月より1988年12月までの高齢者剖検5,089例のうち無病変胃1,895例について，胃粘膜，胃壁動脈の肉眼および組織所見を検討し，その年次的推移を調べて以下の結論を得た．①胃粘膜の腸上皮化生は，1967年より1988年までの22年間に，男女とも漸減の傾向を示した．粘膜の萎縮性変化は女性においては漸減傾向を示したが，男性では有意差はなかった．①胃壁動脈硬化症は，加齢と共に増強するが，幽門腺領域では軽度で，胃底腺領域では高度であった．年次的には，調査した1979年から1988年までの期間では一定の傾向はみられなかった．①わが国の食事内容の欧米化が腸上皮化生の減少傾向と関連があると推定された．

　Using 1,895 autopsy cases with full size of the stomach, time trend of the incidences of some gastric mucosal changes were studied in recent 22 years period. Results are as follows:

　1) The severity of the intestinal metaplasia of the gastric mucosa showed a decreasing tendency year by year. But the mucosal atrophic changes have not a definite tendency with the severity.

　2) The grade of arteriosclerosis in the gastric wall has increased with aging, and was higher in the fundic gland area than the pyloric.

　3) In recent 25 years period, the rate of nutritive elements in Japan has showed a dramatic reversed tendency in relation of cereals to meat foods.

　4) From the tendency of decreasing intestinal metaplasia, the gastric cancer in Japan is expected to have in future the same incidence and characteristics as those in Europe.
