要旨　目的：表面隆起型胃腫瘍の異型度別にみたX線・組織所見の特徴を明らかにする．対象と方法：内視鏡的一括切除を行った胃粘膜内病変のうち，表面隆起型形態を呈した91病変（腺腫 21，低異度分化型癌 31，高異型度分化型癌 39）を対象とし解析を行った．結果：（1）高異型度癌と低異型度癌の差異に関して，高異型度癌は低異型度癌に比べ杯細胞化生が少なく，劣勢分化度が中～低分化，全層主体の発育形式が多く，長径が有意に大きかったが，X線所見では差が認められなかった．（2）低異型度癌と腺腫の差異に関しては，各因子に差がなく類似性が高かったが，低異型度癌では表層の櫛状突起とX線的な微細顆粒模様ありの割合が有意に多かった．（3）微細顆粒模様の組織学的成因は，表層の櫛状突起と長径が独立した因子であった．（4）微細顆粒模様による腺腫と低異型度癌の鑑別診断能は，感度83.6％，特異度57.1％，正診率73.1％であった．結論：表面隆起型低異型度癌は，組織学的に高異型度癌よりも腺腫に類似性が高いが，表層の櫛状突起とX線的な微細顆粒模様の有無が腺腫との鑑別に有用である．

Aims : We tried to clarify the radiographic and histological characteristics of superfietial protruded type gastric neoplasia . Material : 91 lesions of gastric cancer and adenoma clearly verified by histology. Method : We analyzed the clinico-pathological factors related to the degrees of histological atypia（adenoma, low-grade and high-grade of differentiated type carcinoma）. Results : Histological findings ; a meaningful difference between low-grade carcinoma and adenoma was recognized by a comb-shaped projection of the mucosal surface. Radiographic findings ; A meaningful difference was recognized by a fine granular pattern. Conclusions : In radiographic diagnosis related to the degrees of histological atypia, the presence of a fine granular pattern with a comb-shaped projection of the mucosal surface is the most important factor.
