要旨　腸管囊腫様気腫症（pneumatosis cystoides intestinalis；PCI）は，腸管壁の粘膜下または漿膜下に多発性の気腫性囊胞を形成する比較的まれな病態である．基礎疾患，合併疾患の有無により特発性と続発性に分類され，続発性の多くは，消化管疾患，慢性呼吸器疾患，膠原病などとの関係が考えられている．本症は，特徴的な含気性の多発性囊腫であることから画像診断は比較的容易であり，確定診断が得られれば保存的に治療が可能である．まれにfree airを合併することがあり，腹膜刺激症状に乏しい場合は本症を念頭に置く必要がある．

　Pneumatosis cystoides intestinalis (PCI) is a relatively rare condition with multiple gas-filled cysts in the wall of the small or large intestine. While the idiopathic and the secondary etiology are known, the latter often occurs in patients with gastrointestinal disease, chronic respiratory disease and/or collagen disease. Most cases of PCI are asymptomatic and are found incidentally on radiographs of the abdomen. Relatively speaking, we can easily identify this disease based on the characteristic findings of abdominal X-ray, barium enema, and gastrointestinal endoscopy.
