早期胃癌や胃炎などとの鑑別が必要な胃MALTリンパ腫―X線診断を中心に

Radiographic Features of Gastric Mucosa-associated Lymphoid Tissue Lymphoma

要旨　最近5年間に経験した胃MALTリンパ腫19例のX線像を検討した．19例中17例が表層型，2例が腫瘤型に分類された．さらに表層型17例は限局性陥凹の形態を呈した早期胃癌類似型（限局型）5例と広範な顆粒状粘膜パターンを認めた胃炎類似型（広範型）12例の2型に分類可能であった．胃炎類似型のほとんどの症例は，X線所見上，胃小区の乱れに加えて小バリウム斑，ニッシェ，粘膜集中などの多彩な所見を伴っていた．本症のX線診断に際してはこれらの特徴を念頭に置く必要がある．

　To clarify the radiographic features of gastric mucosaassociated lymphoid tissue lymphoma, we reviewed 19 patients, comprised of 17 patients with superficial-type lesions and two patients with mass-type lesions. Among the 17 patients with the superficial type, five patients had localized depressions mimicking 0 IIc type early gastric cancer. The remaining 12 patients had gastritislike lesions whose radiography revealed diffuse granular mucosa with multiple erosions, ulcers and convergencies.
