要旨　カプセル内視鏡は，従来のチューブ型の内視鏡とは全く異なり，被検者が自分でカプセル内視鏡本体を飲み込むだけで検査ができる新しい画像診断法である．現在，小腸を対象とした画像撮影専用タイプ（PillCam^TM SB，GIVEN Imaging社）が最も普及していて，その最大の適応は原因不明消化管出血である．その中で一番多くみつかるのは小腸のangiodysplasiaで，Crohn病，ポリープや腫瘍，NSAIDsによる粘膜傷害などもみつかっている．現在の機種では生検などの処置ができないが，ダブルバルーン式小腸内視鏡との組み合わせによって，より有効な小腸出血の診断治療が可能になる．

　Wireless capsule endoscopy (CE) is a swallowable new diagnostic tool for the gastrointestinal (GI) tract. PillCam^TM SB (GIVEN Imaging, Israel) is widely used throughout the world and its most suitable indication is obscure GI bleeding. CE detects angiodysplasia (most frequent), Crohn's disease, polyp, tumor and mucosal injury caused by NSAIDs in the small intestine. The combination of PillCamTM SB and double balloon endoscopy could contribute to the diagnosis and treatment of small intestinal bleeding.
