◆要旨：【目的】当院における胃切除既往症例に対する腹腔鏡下胆囊摘出術（laparoscopic cholecystectomy：以下，LC）の手術成績を検討し，胃切除既往症例に対するLCが許容される術式か検討する．【対象と方法】2016年4月から2024年3月の8年間に当院で施行されたLC 2,228例のうち，胃切除既往症例45例を対象とし，背景因子と手術成績を検討した．【結果】胃切除既往症例の原疾患は胆囊結石症22例，急性胆囊炎22例，胆囊腫瘍1例で，手術時間中央値は139分，術中出血量中央値は40mL，開腹移行は8例（17.8％），Clavien-Dindo分類GradeⅡ以上の術後合併症は6例（13.3％）であった．【結語】胃切除既往症例に対するLCは胃切除既往のない症例と比較し，開腹移行率は高いものの，許容される術式であると考えられた．
［Purpose］We aimed to investigate the outcome of laparoscopic cholecystectomy（LC） in patients with a history of gastrectomy.［Patients and Methods］A total of 45 patients with a history of gastrectomy undergoing LC from April 2016 to March 2024 in our hospital were enrolled in this study. Background and surgical outcome of these patients were investigated.［Results］LC was indicated for gallbladder stone in 22 patients, acute cholecystitis in 22 patients, and gallbladder tumor in 1 patient. Median operation time was 139 min, and median intraoperative blood loss was 40mL. Eight patients（17.8％） required conversion from LC to open cholecystectomy. Postoperative complications greater than gradeⅡof Clavien-Dindo classification developed in 6 patients（13.3％）．［Conclusion］LC in patients with a history of gastrectomy seems to be an acceptable procedure despite a higher conversion rate to open cholecystectomy compared to patients without any history of gastrectomy.
