 Aspiration prevention surgery is indicated for severe dysphagia unresponsive to conservative therapy. We retrospectively reviewed nine patients who underwent glottic closure(GC, n=6) or laryngotracheal separation(LTS, n=3) at Tokuyama Central Hospital between 2019 and 2024. No severe complications occurred, and aspiration was effectively prevented in all cases. One GC patient achieved a tracheostomy tube-free state, while others required continued cannulation. LTS was selected in pediatric cases for its reversibility but was associated with secretion pooling. GC provided more reliable aspiration prevention but resulted in irreversible voice loss. Both procedures are safe and effective, and surgical choice should be tailored to patient background and family preferences.
