はじめに

　耳下腺転移を生じる頭頸部領域の疾患として，顔面領域の皮膚癌が頻度として高いことが報告されている^1）。頭頸部領域外の悪性腫瘍が耳下腺転移を生じることは非常に稀である。われわれが渉猟しえた限り，本邦では本症例を含めて5例のみである^2〜5）。今回われわれは耳下腺腫瘍として切除を行ったのちにEGFR変異陽性非小細胞性肺癌（肺腺癌）の耳下腺転移と判明した症例を経験したので報告する。

　In this case report, we described a case of parotid metastasis from EGFR mutation-positive non-small-cell lung cancer. A 71-year-old woman was referred to our department for the evaluation of swelling in the right parotid region. Fine-needle aspiration cytology and MR imaging findings suggested the possibility of benign tumors such as myoepithelial tumors or pleomorphic adenomas. Subsequent histological evaluation of the pulmonary nodule led to a diagnosis of pulmonary adenocarcinoma. The parotid lesion was excised for histological diagnosis, and the final diagnosis revealed metastasis from EGFR mutation-positive non-small cell lung cancer.
