要旨●上部消化管内視鏡検査（EGD）における十二指腸観察法はいまだ標準化されていない．筆者らは十二指腸の7部位を系統的に撮像する7PR（seven pictures rule）を考案し，その実装性と腫瘍検出能を前向き観察研究で検討した．7PRは81.1%の完遂率を示し，要治療腫瘍検出率は0.84%であった．腫瘍は下行部乳頭対側に多く，水平部では認めなかった．平均観察時間は53.1秒であり，通常径内視鏡は超細径内視鏡に比して完遂率が高かった．さらにアンケート調査でも7PR認知が腫瘍検出増加に寄与する可能性が示唆された．7PRは実臨床で実行性が高く，観察の定型化に有用な手順である．

　Although esophagogastroduodenoscopy（EGD） is widely performed in clinical practice, no standardized protocol for duodenal observation has been established so far to our knowledge. We developed the Seven Pictures Rule（7PR）, a systematic photo-documentation protocol consisting of seven predefined duodenal landmarks, and prospectively evaluated its feasibility and diagnostic performance for detecting duodenal neoplasms.

　The completion rate of the 7PR protocol was 81.1%, and the detection rate of neoplasms requiring treatment was 0.84%. Most lesions were identified on the contra-area of the ampulla in its descending part, whereas no neoplasms were detected in the transverse part. The mean observation interval was 53.1s. In addition, standard-caliber endoscopes achieved significantly higher completion rates than ultra-thin endoscopes. Furthermore, a subsequent questionnaire-based survey suggested that familiarity with the 7PR protocol may contribute to improved duodenal neoplasm detection.

　These findings indicate that the 7PR is a feasible and clinically applicable standardized approach for duodenal observation during routine EGD, and it may constitute a useful quality indicator for duodenal examination.
