Abstract Purpose:Lacrimal sac diverticulum(LSD) is a rare disorder characterized by outpouching of the lacrimal sac;and its association with tumors has rarely been reported. Herein, we describe an extremely rare case of LSD associated with mucosa-associated lymphoid tissue(MALT) lymphoma.

Case:A 66-year-old woman experienced recurrent dacryocystitis since June of year 2023 and had undergone repeated lacrimal sac punctures at a previous hospital. During the first visit to our department, a mass was noted in the right lacrimal sac region, and unenhanced CT revealed a 23-mm cystic lesion. In October of the same year, an endonasal dacryocystorhinostomy was performed for right nasolacrimal duct obstruction, and the postoperative course was favorable. However, three months later, swelling recurred in the lacrimal sac region. Lacrimal irrigation revealed good passage, and a LSD was suspected. Eight months later, swelling recurred, and excision of the LSD was performed in October 2024. A cystic lesion containing dacryoliths was excised, and histopathological examination confirmed a diagnosis of MALT lymphoma. PET/CT revealed no abnormal uptake outside the right lacrimal sac. The patient was followed up without systemic treatment at the hematology department, and no recurrence has been reported to date.

Conclusions:This case suggested that MALT lymphoma-induced obstruction might have contributed to the formation of LSD. When a LSD is diagnosed, the possible coexistence of neoplastic lesions should also be considered.
