Abstract Purpose:To report a case of conjunctival malignant melanoma effectively treated with subconjunctival injection of interferon beta(IFN-β).

Case:A 91-year-old female presented to our hospital for treatment of an elevated tumor in the lower palpebral conjunctiva of the right eye. She had a history of hypertension, diabetes, and nephrotic syndrome.

Findings and Clinical Course:A tumor, 15 mm in diameter, with bleeding in the lower palpebral conjunctiva was observed in her right eye. Extended resection was recommended, however the patient declined. Tumor biopsy revealed malignant melanoma upon histological diagnosis. No other abnormality was identified in other organs. Radiotherapy(28 Gy/14 Fr)was performed to control bleeding and was followed by subconjunctival administration of IFN-β(3 million units)weekly. Bleeding ceased after one dose;tumor size was reduced after three courses, and was almost completely regressed after seven courses. Vision loss occured as a side effect, and so the dose was reduced to one half and the dosing interval was extended. Neither local recurrence nor metastases was observed during the treatment. After 8 months, treatment was abandoned due to impairment of renal function.

Conclusion:Subconjunctival injection of IFN-β was effective in reducing the size of conjunctival malignant melanoma. But IFN-β may cause side effects not only in the eye but also in other organs, so it is necessary to calibrate the dose and dosing interval carefully.
