Abstract Purpose:To report a case of an iron conjunctival foreign body retained for 4 years after a lawn trimmer injury, discovered during an examination for conjunctival tumor.

Case:A 67-year-old woman sustained a conjunctvial laceration and vitreous hemorrhage in her left eye caused due to lawn trimmer injury. She had been asymptomatic for three years. Four years post-injury, a conjunctival tumor was observed in the nasal bulbar conjunctiva of the left eye. The patient was referred to the Kagoshima University Hospital for further evaluation.

Findings:A hyperemic granulomatous tumor(10×5 mm) was identified in the nasal conjunctiva of the left eye. Computed tomography(CT) revealed a high-density lesion(8×4 mm) in the same area. Suspecting granulation tissue associated with a conjunctival foreign body, tumor resection was performed under local anesthesia. A 6×2 mm iron fragment was found within the mass surrounded by connective tissue. Histopathological analysis revealed hyalinized collagen fibers with neovascular capillaries and lymphocytic infiltration.

Conclusion:Foreign bodies resulting from ocular trauma can be concealed by hemorrhage, tissue swelling, or eye movements;therefore, imaging studies are essential to confirm their presence. In addition, conjunctival foreign bodies may gradually form and manifest as masses due to physical stimulation, such as eye movements and blinking. Long-term observation is crucial for detecting and managing residual foreign bodies.
