Abstract Purpose:Sparganosis is an infection caused by the plerocercoid larvae(spargana)of various diphyllobothroid tapeworms belonging to the genus Spirometra. Although subcutaneous sparganosis is common, there are few reports of ocular sparganosis in Japan. Here, we reported a rare case of ocular sparganosis.

Case:The patient was a 33-year-old male. Swelling of the right eyelid appeared and he was diagnosed with conjunctivitis of the right eye by local practice in July 2017. Because the symptoms did not improve, he visited Kagoshima University Hospital in October 2018. A migratory swelling was noted in the right upper eyelid, and MRI showed inflammatory findings in the right upper rectus and medial rectus muscles. The patient was diagnosed with orbital inflammation due to IgG4-related disease and was treated with oral steroids. However, the symptom was exacerbated and remitted repeatedly. A white lesion was found on the conjunctiva and the lesion was removed in June 2020. The right eyelid swelling was then improved. Histopathological examination showed that the removed white lesion was a Spirometra erinaceieuropaei.

Findings and Clinical Course:Ocular sparganosis are rare and often misdiagnosed because of the similarities to idiopathic orbital inflammation in symptoms. Ocular sparganosis should be remembered when diagnosing repeated and steroid-resistant eyelid swelling and exophthalmos.
