Abstract A 65-year-old female was referred to our hospital with ulcers on the right buttock. A skin biopsy specimen from the buttock showed infiltrates of massive large atypical cells that were positive for CD3, CD4, CD30, CD56 and negative for ALK. A PET-CT scan showed multiple FDG uptake throughout the body. We made the diagnosis of ALK-negative systemic ALCL, which is known to have a very poor prognosis. The patient died 45 days after the first visit. CD56 positivity is known to be an indicator of poor prognosis in ALCL. As seen in previous studies, our case had a rapidly progressive course.
