消化管感染症の病理診断

Pathological diagnosis of gastrointestinal infectious disease

要旨

感染症診断の分野において通常の病理検査には原理上一定の限界が存在し，腫瘍診断のように単独で診断確定まで至ることは稀である。ただし，十分な臨床情報の提供と適切な検体採取を前提として，ある程度確定的，もしくは治療に有益な情報が得られる症例は存在する。消化管領域の感染症の中で病理検査が一定の役割を果たせる可能性のある各疾患について病理医の視点から概説する。臨床医には病理検査の有効性と限界をよく踏まえたうえで適切な検査依頼，結果の解釈を行うことが望まれる。

　In the field of infectious disease, pathological examination has certain limitations and rarely can result in a definitive diagnosis alone, unlike in diagnosing tumors. However, with sufficient clinical information and appropriate specimens, there are some cases in which an almost-definitive diagnosis or useful information for treatment can be obtained by pathological examination. In this article, we outline the infectious diseases of the gastrointestinal tract, in which pathological examination may play a role, from the pathologist’s point of view. Clinicians should ideally make appropriate requests and interpret the results, based on an understanding of the validity and limitations of pathology.
