The pancreas is composed of diverse tissue lineages, and various pancreatic tumors or carcinomas arise from those cell lineages. Solid pancreatic tumors include pancreatic ductal adenocarcinoma, acinar cell carcinoma and pancreatic neuroendocrine neoplasms. In addition, pancreatic cystic lesions can become malignant. In this article, we review the classification and epidemiology of pancreatic cystic lesions as well as their potential for carcinogenesis. First, pancreatic cysts are classified into non-epithelial cysts such as pseudocysts, non-neoplastic cysts such as lymphoepithelial cysts or epithelioid cysts, and neoplastic cysts such as IPMN, MCN, SCN, and SPN. NET can degenerate and present cystic findings. Recently the opportunities to find pancreatic cystic lesions have increased due to advances in diagnostic imaging modalities. The malignant potential of neoplastic cysts varies, and it is essential to update their clinical outcomes.
