 Work-style reforms have led to shorter working hours for physicians, while the aging population is reshaping the structure of healthcare. In this environment, operating rooms must be managed with greater efficiency and flexibility.

 At our hospital, all staff recognize that the OR is a shared facility. Surgeons, anesthesiologists, and nurses cooperate based on clear rules. The anesthesiology department follows a “never refuse” policy, enabling quick surgical responses and building trust with surgeons. Even when it is difficult for an anesthesiologist to accept a request for participation in a case, alternative solutions are proposed to maintain collaboration.

 With nearly 30% of our hospital’s surgeries being emergencies, we distribute elective surgeries evenly throughout the week. In coordination with surgeons, we have also proposed outpatient clinic adjustments. As a result, most of the scheduled surgeries are completed before the evening, leaving room for emergency care at night, while also supporting workstyle reform.

 Surgery requires both human and material resources, which are limited. Reducing the length of time that patients spend in the OR is key to maximizing the use of material resources. By leading efforts to streamline surgical procedures, anesthesiologists can contribute to sustainable healthcare in an era of population aging and workforce decline.
