 Double-lumen tubes(DLTs)have been used for one-lung ventilation during thoracic surgery. A left-sided DLT is preferable in most cases because of its safety and ease of use. It is important to maintain the proper position of a DLT for successful one-lung ventilation;malposition of a DLT can lead to life-threatening complications. The use of a fiberoptic bronchoscope is the key to assess the position of a double-lumen tube.

 In this article I describe the structure of DLTs, the use of a left-sided DLT, the selection of a DLT’s size, tracheal and bronchial intubation techniques, proper positioning of a left-sided DLT, confirmation of proper DLT placement, common problems associated with the malposition of a DLT, and the use of a fiberoptic bronchoscope during thoracic anesthesia.
