はじめに

　分離肺換気とは左右の肺に異なる換気を実施することであるが，多くの場合は片肺を換気しない一側肺換気（one-lung ventilation：OLV）を示す。術前に心肺機能がOLVを許容するか評価するとともに，患者の上・下気道の形態について評価し，気道管理計画を立案する。上・下気道の形態と手術部位に応じて，経鼻・経口・気管切開孔などの気道確保経路と使用する気道管理デバイスを選択する。換気能力とガス交換能が極度に低下し，OLVの維持が困難な症例は，葉ブロック，間欠的両肺換気や持続気道陽圧（continuous positive airway pressure：CPAP）システムの併用，体外式膜型人工肺（extracorporeal membrane oxygenation：ECMO）などが必要である。本論文では特殊症例として，気管切開孔からの分離肺換気，気管気管支の形態と分離肺換気のデバイス選択について解説したあと，気管断裂症例へのダブルルーメンチューブ（double-lumen tube：DLT）の留置方法，気管壊死症例に対する左気管支へのシングルルーメンチューブ（single-lumen tube：SLT）挿管による気道確保，気管支皮膚瘻症例に瘻孔を閉鎖しつつ陽圧換気を実施する方法について提示する。

　When one-lung ventilation（OLV）is required for special cases, the patient’s cardiopulmonary function should first be assessed to determine whether the patient can tolerate OLV. When the patient’s cardiopulmonary function can tolerate OLV, his or her airway morphology is evaluated in detail, and airway management plans are drawn up. The airway morphology and the surgical procedure affect the selection of the intubation route and the airway management device. For differential lung ventilation from the tracheostoma, the device selection takes into account the diameters and shapes of the tracheostoma, trachea, and bronchus, and the ventilation side. For tracheal bronchus, a double-lumen endobronchial tube can be used if the distance from the tracheal bronchus to the carina is≤2 cm, and if the distance is＞2 cm, a combination of a single-lumen endotracheal tube（SLT）and an endobronchial blocker（EBB）is suitable. Right OLV is performed by blocking the left bronchus with an EBB, and left OLV is performed by intubating an SLT to the left bronchus. If the pulmonary blood flow distribution is normal, left OLV is often advantageous in OLV by providing bronchial intubation with an SLT, and the diameter and length of the left main bronchus should be measured before anesthesia is administered.
