 Bronchial blockers(BBs)are slender devices equipped with a balloon at the tip, used in conjunction with endotracheal tubes to achieve lung isolation. Although double-lumen tubes(DLTs)are typically the first choice in procedures that require lung isolation, BBs offer a valuable alternative in cases that involve a difficult airway, emergency surgeries, and pediatric patients for whom the use of a DLT may be challenging. BBs are particularly advantageous due to their compatibility with oral, nasal, and tracheostomy tubes as well as their utility in patients whose cases require postoperative ventilation or selective lobar isolation. BBs also have limitations, such as reduced suction capability and the need for fine bronchoscopic techniques. This article outlines the types and structures of the available BBs, the indications for BB use, insertion procedures, and troubleshooting strategies. Key considerations regarding effective lung collapse are also highlighted, including the disconnection technique. Right-sided block challenges and intraoperative complications are addressed. Recent progress such as the integration of electromagnetic navigation bronchoscopy and novel BB designs are also discussed. With clinicians’ appropriate knowledge and skill, BBs serve as a flexible and powerful tool for safe and effective lung isolation.
