緊急手術麻酔におけるポイントオブケア超音波

Point of Care Ultrasound in Emergency Surgical Anesthesia

はじめに

　緊急手術を受ける患者は手術前に必ずしも必要な検査や治療が行えず，麻酔管理に支障を来しうる。しかし，麻酔科医自身がリスクを評価し対応すればその後の管理が容易となる。今回は，緊急手術時に迅速に患者をアセスメントするツールとしてポイントオブケア超音波（point of care ultrasound：POCUS）を紹介する。なお，周術期の体位制限や時間的猶予など通常より難易度が上がるため，異常所見を見抜くには日ごろよりPOCUSに慣れ親しむ必要がある。

　In emergency surgery, anesthesiologists can perform safe anesthetic management by using point of care ultrasound（POCUS）for risk assessments and diagnoses. In the operating room, POCUS can be used as a tool for the rapid assessment of the airway, stomach, lungs, heart, trauma, and deep vein thrombosis（DVT）. Given the increasing recognition of the importance of POCUS, we anesthesiologists can contribute to the safety of patients undergoing surgery by improving our POCUS knowledge and skills.
