術前非機能性腫瘍とされた大動脈周囲の結節が術中異常高血圧から褐色細胞腫との診断に至った1症例

A Case Diagnosed as Not Non-functional Tumor but Extra-adrenal Pheochromocytoma of Nodule Around Aorta According to Intraoperative Abnormal Hypertension

要旨

　術前に腹部のコンピュータ断層撮影（computed tomography：CT）画像と脳出血の既往により異所性褐色細胞腫を疑う所見があったにもかかわらず，非機能性腫瘍と診断し精査を怠り，開腹肝切除施行時に異常高血圧と低血圧を来し，術後の精査で異所性褐色細胞腫と診断された症例を経験した。疑う所見がある場合には術前に外科医と相談し精査を行うべきであると考えられた。

　We have experienced a case diagnosed with extra-adrenal pheochromocytoma who exhibited hypertensive crisis during open liver resection although this patient had suspected nodule of abdominal CT scan and previous symptom of brain bleeding clinically. We should discuss with our surgical team and conduct examinations in detail preoperatively in a case with suspected clinical problems.
