要旨

　パラガングリオーマを合併した75歳の男性患者に対して3回の全身麻酔を行った。全身麻酔単独での1回目で，術中高血圧クリーゼが疑われ，術後にパラガングリオーマと診断された。2回目以降，術前に血圧コントロールと循環血漿量補正を行い，区域麻酔併用全身麻酔とすることで，深刻な周術期合併症を生じなかった。

　A 75-year-old man with paraganglioma（PGL）received general anesthesia three times. Hypertensive crisis was suspected due to abnormal hypertension and electrocardiogram abnormalities during his first surgery, which was performed under general anesthesia alone. PGL was diagnosed after the first surgery. The second and third surgeries were performed under general anesthesia combined with regional anesthesia after preoperative blood pressure control and circulating plasma volume correction. Under this regimen, the patient experienced no serious complications during the second or third perioperative periods.
