 Even if scar contracture develops after the epithelialization of a burn wound, mild cases can often be expected to improve with the use of Eclar Ⓡ plaster. In principle, as long as there is no marked functional limitation, it is desirable to first observe the course for ~6 months. The indication for surgery can then be determined based on the subsequent progress. On the other hand, if ~3 months have passed since the burn injury and the inflammation and contracture continue to worsen without signs of improvement, reconstructive surgery should be planned at an early stage. Spontaneous resolution is unlikely in this state of persistent strong inflammation, and even if the inflammation eventually subsides, there is a high risk of the development of severe scar contracture or joint contracture. In post-burn reconstruction, it is advisable to formulate the reconstructive strategy in accordance with the Reconstructive Ladder. It is essential to understand the advantages and disadvantages of each method — skin grafts, local flaps, distant flaps, expanded flaps, supercharged flaps, and free flaps — and to select the most appropriate option according to the individual case.
