Two Cases of Buttock Skin Defects Closed Using a Pacman flap

はじめに

　Pacman flapとはV-Y前進皮弁を応用した皮弁で，ゲームのキャラクターである「パックマン」に皮弁の形が似ていることからAokiら 1）が呼称した術式である。本皮弁は審美的な問題が大きい顔面や頸部の再建に用いた報告が多い 1）。

　今回われわれは，殿部の比較的大きい皮膚欠損に対してPacman flapを用いる，あるいは組み合わせることによって良好な結果を得た2例を経験したため報告する。

　For skin defects in the buttock region, reconstruction using V-Y advancement flaps and transposition flaps such as Limberg flaps are common procedures. We report a case in which a pilonidal sinus in a 33-year-old man was reconstructed using a unilateral Pacman flap, and a case in which a sacral pressure ulcer in an 89-year-old woman was reconstructed using V-Y advancement flaps and Pacman flaps on both sides. There were no problems such as postoperative recurrence or dehiscence in either patientʼs case, and their outcomes indicate that the Pacman flap can be useful for large skin defects in the sacral region.
