Reconstruction of the Large Scalp Defect by Free Flap Transplantation

I．はじめに

　広範囲頭皮欠損の修復は極めて困難なことが多く，以前は数回の手術，長期間の入院を要したにもかかわらず，不成功に終わることが少なくなかった．最近10年間に急速に発達した微小血管吻合による遊離組織移植（free　flap）によれば，大きな頭皮欠損も1回の手術で修復可能となった．著者らはfree flapによる頭皮欠損修復を6例に経験し，全例に好結果を得ることができたので，その症例の一部を紹介する．

The reconstruction of a large scalp defect is extremely difficult on many occasions and repeated operations or a long-term hospitalization is often required. In addition, unsatisfactory results are not unusual.

However, it is now possible to perform reconstruc-tion by a one-stage operation of free tissue trans-plantation (free flap) using microsurgical vascular anastomosis which has rapidly developed in the past ten years.

We performed reconstructions applying latissimus dorsi musculocutaneous, greater omentum, groin andscapular flaps.
