遊離腓骨皮弁採取後に生じたclaw toe変形に対し長母趾屈筋腱Z延長術を施行した2例

Successful Experience with Z-Extension of the Flexor Hallucis Longus Tendon for Claw Toe Deformity after Mandibular Reconstruction with a Vascularized Fibula Grafts

はじめに

　遊離腓骨皮弁は，骨の再建が必要な多くの臨床場面で用いられる皮弁である。皮弁採取後の採取部の主な合併症には，感染などによる創傷治癒遅延や腓骨神経障害および歩行障害などがある。母趾の底屈変形は比較的まれな合併症とされ，原因として皮弁採取の際の長母趾屈筋（flexor hallucis longus muscle：以下，FHL）の何らかの損傷や栄養血管の操作による虚血もしくは術後のコンパートメント症候群の発生などが考えられている。この合併症に対して，FHL腱の延長術・切離術が有効である症例の報告も散見される 2）3）。今回われわれは遊離腓骨皮弁採取部に生じたclaw toe変形に対して，腱延長術を行い改善を認めた2例を経験したので報告する。

　Flexion deformity of the great toe is one of the complications of vascularized fibula grafts. We have treated two patients with flexion deformity of the great toe after a free vascularized fibular graft was harvested from the ipsilateral leg. In both cases, Z-extension of the flexor hallucis longus muscle improved the deformity. The maximum follow-up was 34 months, and full extension of the toes of both patients was achieved at the final follow-up.
