授乳期の乳輪乳頭の疾患—助産師の視点から—

Trouble with Nipple and Areola during Breastfeeding

はじめに

　乳房・乳頭は女性の自己概念に深くかかわっており，女性としてのイメージやセクシャリティのシンボルの1つとしてその自尊感情に大きく影響している。著者は助産師として，母乳育児の観点から乳輪乳頭の問題にかかわってきた。また，私自身の臨床・研究領域は母乳育児支援であることから，乳房・乳頭のシンボリックな意味については別の専門家に譲ることとする。本稿では，授乳期における乳輪乳頭の形態と機能およびそれらに関連するトラブルについて概説する。

　Nipple malformation may influence breastfeeding, however, if the nipple and areola are sufficiently flexible and extensible to breastfeed, that influence is not necessarily negative.

　Surgical treatment of a breast or nipple for an inverted nipple or big nipple sometimes results in milk duct damage that has a significant negative effect on breastfeeding. For this reason, surgical intervention should be avoided as much as possible.

　For the same reason, if surgery is necessary, great care must be taken to avoid damaging the milk ducts during surgical treatment for mastitis or cosmetic breast surgery.
