 One of the most common postoperative complications of congenital heart surgery is pleural effusion, the reported incidence of which is 0.25 %-9 %. Lymphatic vessels are rich around the ductus arteriosus and aorta, and both the injury of a thoracic lymph duct during surgery and an elevation of systemic venous pressure due to right heart bypass can result in pleural effusion. The pathophysiologic features of chylothorax include respiratory disorder, infection, loss of nutrients, and growth disturbance.

 We report the case of a patient who exhibited intractable pleural effusion after the Fontan procedure for hypoplastic left heart syndrome. The patient was initially treated by steroids, octreotide, coagulation factor and milk contains medium-chain triglycerides, but these methods failed. We evaluated the lymphatic flow by using Lipiodol Ⓡ lymphangiography, and although we were unable to locate the leakage, the drainage of the pleural effusion decreased immediately after the post-lymphangiography examination, which lead to a successful outcome.
