リピオドールⓇリンパ管造影検査で改善した小児先天性心疾患術後難治性胸水の1例

A Case of Intractable Pleural Effusion after Congenital Heart Surgery for Hypoplastic Left Heart Syndrome, Cured by Lipiodol Ⓡ Lymphangiography

はじめに

　先天性心疾患の外科治療の術後合併症として比較的頻度の高いものの1つに乳び胸があり，0.25～9 ％に合併するといわれている 1）。動脈管や下行大動脈周囲にリンパ管は豊富に存在し，手術操作による外傷性リンパ管損傷や，術後に静脈圧が亢進することで，リンパ液が胸腔内に漏出することが原因となる。術後難治性乳び胸水により長期間持続的に大量のリンパ性胸水が漏出することで，感染症や呼吸障害，栄養障害，成長障害につながり，致死的となることがある 2）3）。

　今回，左心低形成症候群に対して，Fontan術後に外傷性リンパ漏を来たし，内科的治療が無効であった症例に対して行った鼠径リンパ節穿刺によるリピオドール Ⓡ（油性ヨード化ケシ油脂肪酸エチルエステル：ゲルべ・ジャパン社）リンパ管造影検査が胸水減少に寄与した1例を経験したため報告する。

　One of the most common postoperative complications of congenital heart surgery is pleural effusion, the reported incidence of which is 0.25 ％-9 ％. Lymphatic vessels are rich around the ductus arteriosus and aorta, and both the injury of a thoracic lymph duct during surgery and an elevation of systemic venous pressure due to right heart bypass can result in pleural effusion. The pathophysiologic features of chylothorax include respiratory disorder, infection, loss of nutrients, and growth disturbance.

　We report the case of a patient who exhibited intractable pleural effusion after the Fontan procedure for hypoplastic left heart syndrome. The patient was initially treated by steroids, octreotide, coagulation factor and milk contains medium-chain triglycerides, but these methods failed. We evaluated the lymphatic flow by using Lipiodol Ⓡ lymphangiography, and although we were unable to locate the leakage, the drainage of the pleural effusion decreased immediately after the post-lymphangiography examination, which lead to a successful outcome.
